Here an example of the perfect holding a camera with a linger lens. I took at that photo in 2010 at the Winter Olympics, in Vancouver, Canada. She was taking photos of people swinging from the ZIP line at the Vancouver Art Gallery.

Here's an example of the perfect hold of the camera with a longer lens. The right hand holds the grip, the index on the shutter button. The three fingers on the the grip in a “relaxed” way. In the palm of the left hand, she hold the bottom of the lens. She uses her thumb and her middle finger to zoom.
